Natural Parent Information Questionnaire
The information you put on this form tells us about you and about the birth father. The purpose is so the adoptive family you choose will know something about you, and so the baby can be told about you when s/he gets older. Some of the questions may not apply, so just do the best you can. (We realize that you may not know the answer to every question - if you don't know, just skip it.)
Authorization for Release of Health Information Form
When you sign this form, you give us permission to talk to your doctor, and to get copies of your medical records. It will also allow us to get copies of the baby's hospital records, which we will need to do to complete the adoption.

Medical Release Father
These questions ask about your own medical history, and that of the father, and of your families. The purpose of gathering this information is so that if the baby ever gets sick, the doctors will have a better idea if the illness is something the child inherited or not. This form appears more complicated than it is. Take your time and work through the form filling in whether you, or the baby's father, or anyone in either of your families, has or had any of the conditions listed. Most of the conditions will not apply, so, again, do your best. And we realize you may not know - if so, just leave it blank.
American Indian Background Form
This form asks you for any information about Native American heritage you and the baby's father may have. (Specific laws apply in the case of an adoption of an American Indian child.)
Natural Mother’s Preferences Questionnaire
This form is sent to the hospital before the baby is born so that the hospital staff will be prepared to comply with your wishes, and will know whether you want to have any contact with the baby while you are in the hospital, etc. Please think about how you would like things handled, and fill in the information. (You may also change this information when you check in at the hospital, if you like.)
Statement Concerning Legal Representation
In the usual case, we normally represent only the adoptive parent(s). However, you may have your own lawyer to advise you about adoption, or represent you in any negotiations or proceedings in connection with the adoption. We encourage you to be fully informed of your rights, obligations, and options. The adoptive parent(s) chosen can pay the legal fees for you, under certain circumstances. If you have any questions about this, please ask us.
